The Hong Kong Society for Rehabilitation (hereunder as ‘HKSR’) is a pioneer in rehabilitation dedicated to providing holistic and personalized services to empower persons with disabilities, chronic illnesses or challenges of aging to lead fulfilling lives, and to advocate for an inclusive society.  We are a non-governmental organization receiving subvention from the government and funds from private donors and other sources.  We invite people with the heart and passion to join us in realizing our social mission.

 

District Health Centre (DHC) aims to provide primary healthcare services including health promotion, assessment and patient empowerment program to enhance the public awareness on disease prevention and the capacity in the self-management of health through the public-private partnership.

 

Community Health Officer (Eastern District Health Centre)

Key responsibilities:

  • Deliver health education activities and community programs to promote health awareness, disease prevention, and the importance of peer support and social inclusion in DHC.
  • Provide primary healthcare services, community rehabilitation, and self-management programs to enhance the well-being of persons with disabilities, chronic illnesses, and their caregivers.
  • Facilitate mutual support groups and volunteer groups.
  • Assist in delivering community health or rehabilitation support programs.
  • Support other duties or ad hoc tasks as assigned by supervisors.

 

Requirements:

  • Degree holder in health-related studies such as public health, community health practice, nutrition, gerontology, or sport science.
  • At least two years of work experience in the community, primary healthcare, or in serving people with disabilities, chronic illnesses, their caregivers, and related community organizations is preferred. Experience in related health and social services, such as as a Health Care Assistant, will be an advantage.
  • Knowledge of public health, exercise, health management, chronic disease management, mutual support services, volunteer services, and e-service delivery.
  • Strong interpersonal, communication, and time management skills.
  • Mature, self-motivated, and innovative.
  • Good team player in a multidisciplinary team.
  • Good command of both Chinese and English.
  • Shift duty, including 1–2 night shifts per week until 9:00 p.m., is required.
  • Immediate availability is preferred.

The successful candidate will normally be appointed on contract terms.  Renewal of contract will be subject to the service/operational needs of the department and the candidate’s performance.
