Assistant Manager (Community Service) – Registered Nurse
Key Responsibilities:
- Support the Manager in planning, implementing, monitoring and evaluating the Home-based Chronic Disease Management Service for the Elderly (CDME) to ensure clinical safety, service quality and alignment with agency strategic directions.
- Provide nursing assessment and evaluation, care planning, intervention, ongoing monitoring and health education for clients in community and home-based settings, with an emphasis on Tai Po CDME cases and home-based chronic disease management.
- Provide health consultation and follow-up support to older people and caregivers on chronic disease management, medication management, health monitoring, disease prevention and daily health routines.
- Coordinate multidisciplinary care and referrals by working with social workers, allied health professionals, community partners, internal teams and healthcare institutions.
- Conduct health education sessions and talks, prepare, review and enhance education materials and assist in promotional activities to raise public awareness of health issues, disease management and daily health routines.
- Organize staff and volunteer training, and community health promotion and education activities to enhance self‑management, health literacy and well‑being among target groups.
- Support service monitoring, including documentation of health assessment records, service statistics, user feedback and reporting requirements.
- Carry out ad hoc projects and tasks assigned by the supervisor.
Requirements:
- Registered Nurse registered with the Nursing Council of Hong Kong, with a valid practising certificate.
- Graduate of tertiary education level or above, with a minimum of 2 years of post-registration and relevant working experience.
- Sound knowledge in health education, chronic disease management and community services will be an advantage.
- Service and client-oriented, mature, with good analytical thinking, communication and presentation skills.
- Good interpersonal skills and able to work both independently and in a multidisciplinary team under pressure.
- Excellent command of both written and spoken English and Chinese (including Putonghua)
- Fresh Graduate will also be considered
Working Locations: West Kowloon
Contract Period: 1 year
